Click below to set up your secure patient portal and book your first visit. New patients are welcome to start with a complimentary 15-minute introductory call, or go straight to a 60-minute consultation if you're ready.
Step 2: Check Your Email
You'll receive a link to set your password and access your portal. Once you're in, take a few minutes to explore. This is where your appointments, documents, and care history will live.
Step 3: Complete Your Intake Materials:
Before your first visit, please fill out all consent forms and health questionnaires in your portal. If there are records, labs, or notes from other providers you'd like us to review, upload them here. The more context we have going in, the more we can do for you from the very first appointment.
